From Mandate to Monday Morning: Coordinating Medi-Cal Care Under CalAIM

California’s CalAIM reforms transformed care coordination into a funded, contracted Medi-Cal benefit. Among many other changes, the reforms also encourage counties to coordinate across behavioral health, managed care plans, housing, probation and the courts on a scale like never before. Which begs the question: How will these reforms impact California care providers’ ability to help those they serve?

In this micro learning session, Netsmart and the San Bernardino County Department of Behavioral Health will demonstrate how Enhanced Care Management (ECM), Justice-Involved Reentry, CARE Court and homeless services can operate on a unified care coordination platform instead of relying on siloed systems for each program like they have in the past. Attendees will learn how policy requirements feed into day-to-day workflows, including shared care plans, data exchange with managed care plans and outcomes reporting.
