By connecting people in need to programs that serve them with dignity and ease, the Findhelp platform enables community organizations, governments and businesses across industries to easily manage and coordinate care. From screening and closed-loop referrals to outcomes tracking and actionable insights, Findhelp is powering a better social safety net. This mission is amplified through Findhelp being embedded into Netsmart CareRecords (myEvolv and myAvatar) and by reaching the communities that Netsmart serves.

Findhelp and Netsmart are leveraging their combined expertise to reduce documentation and streamline efficiency in the social care space – all while capturing patient Social Determinants of Health (SdoH) data. Through the Findhelp technology platform, it’s possible to track updates from staff, patients and community organizations; close the loop on social care referrals; and analyze individual and aggregate data to drive strategic decisions.

This collaboration facilitates seamless workflows by enabling information exchange between systems and offering a single sign-on experience. Additionally, processes are put in place to mitigate duplicative documentation. For example, referrals made in Findhelp display in Netsmart systems, in addition to referral updates made by community organizations in Findhelp flowing to Netsmart. Finally, the systems prioritize privacy and security, which is evidenced by encrypted information and technology that’s HITRUST certified and HIPPA compliant.
