Netsmart and Missouri Coalition for Behavioral Healthcare Announce Expansion of Partnership to Support Statewide Healthcare Home Program

Overland Park, Kan., Sept. 20, 2017Netsmart has been selected by the Missouri Coalition for Community Behavioral Healthcare and its members as the new statewide care coordination and population health management partner to support all members of the coalition participating in the Healthcare Homes program.

In 2016, New Horizons, Truman Behavioral Health, along with Netsmart clients Tri-County, Compass Health and Ozark Center were selected to participate in a year-long pilot program to utilize Netsmart’s cloud-based solution to provide insights into the populations they serve as part of the Healthcare Homes program. The successful outcome of the pilot program led to the selection of the Netsmart population health management solution to support all Community Mental Health Center’s (CMHC) statewide. The solution provides nurse care managers with a tool that offers workflow friendly views of both physical and behavioral health data, as well as proactive alerts and auto-generated tasks to guide care.

“Before deploying the Netsmart solution, all of our data was in disparate systems,” said Rachelle Glavin, clinical director for the Missouri Coalition. “Now we have an aggregate data set that allows us to eliminate many previously manual and duplicative tasks. For example, we are now able to automate the calculation of risk for our population, leveraging both encounter and claims data to create a living risk profile for the attributed population.”

This innovative approach of implementing a statewide population management system across the state of Missouri is a critical step to improving the health of all persons across the state. Through this partnership, healthcare organizations can view more information about the consumer and gain insights into how to treat the whole person, so they can provide the best care possible at lower costs.
