Seven Hills Foundation Selects Netsmart as Care Coordination Platform for Transformative Massachusetts Initiative

Overland Park, Kan., Oct. 5, 2017- Today, Netsmart announced it has been selected as the care coordination and population health management provider for the Massachusetts Care Coordination Network, led by the Seven Hills Foundation. The Community Partners program is part of MassHealth’s delivery reform initiatives to integrate and improve health outcomes for members with complex long-term medical or behavioral health needs.

“The goal of the Community Partners program is to develop coordinated care that meets the needs of individuals with complex needs while reducing the total cost of care” Seven Hills Foundation CEO Dr. Kathee Jordan said. “We chose Netsmart because they have a proven record of providing solutions that improve collaboration, eliminate silos and enable integrated care. The populations we serve have complex needs and our goal is to assist them in navigating the complicated healthcare system so that they have access to services they need throughout the community.”

To create a network of providers, Seven Hills Foundation is partnering with Advocates, Boston Center for Independent Living, Horace Mann Educational Associates (HMEA), BayPath Elder Services and Brockton Area Multi-Services, Inc. (BAMSI). The collaborative will ultimately have access to integrated health data that will enable them to align their own care teams with the program quality measures. Team members will be informed by using rules-driven alerts and notifications proactively when critical events have occurred. The actions of the care team will be driven by best practices and the program quality measures, ensuring standardization across the provider network to provide individuals the right care at the right time in the most appropriate setting.

As a key component of the project, Netsmart will connect these provider organizations to existing primary care, acute care and behavioral health networks to enable integrated care. Regardless of what electronic health record (EHR) platform the organization operates, using interoperability standards, providers will be able to integrate care throughout the network. Taking advantage of a large provider network will enable care coordination to occur more efficiently. The alignment of workflows to program quality measures will ensure the ability to integrate care across provider teams.

MassHealth will require participating organizations to meet certain quality targets, for example follow-up visits after hospitalization for mental illness, preventable emergency department utilization and completion of social services screenings. This pay-for-quality approach will align providers with the state’s overall population health management goals for the program.
