Earlier this year, we participated in the #HIMSS18 Interoperability Showcase where we demonstrated how interconnected systems allow healthcare teams to provide whole person care.
Here’s the scenario: Officer Tim Jones is a 54-year-old police officer with diabetes. He visits his primary care provider where he informs the doctor he has gained weight and has trouble sleeping. At the visit, the doctor noted that his A1C levels are higher than normal. Through interoperability, we can see his journey through various health settings take shape, from primary care to consultations to home care. The sharing and exchange of vital health data across networks to each care provider enables Officer Tim’s care team to make the best clinical decisions to support his journey to wellness, keeping him in the most efficient and lowest acuity setting.
Watch the entire video below or jump directly to our role in Officer Tim’s journey at the 8:38 mark.

At the core of this strategy lies value-based contracting, a payment model that aims to align provider reimbursement with the outcomes achieved by patients. By incentivizing quality care and efficient resource utilization, value-based contracting promotes a more coordinated and effective healthcare system.
