3/08/18 1:00 PM (CST) Ensuring Sustainability and Improving Outcomes in Child Welfare through Value-Based Approaches

Value-based payment models continue to emerge as a focal point across the health care system, especially as they relate to integrated and whole-person care. Many community-based human services organizations, which are historically underfunded and overstrained, view the prospect of implementing transformational models of care and reimbursement as disconcerting and lots of work for little value. But the alternative is not participating in value-based models, which would render services unsustainable and obsolete.

Join Netsmart associate Gary Beckham, solutions strategist, to learn why a value-based approach is essential for improving consumer outcomes and ensuring organizational sustainability, especially in child and family services organizations.

You'll learn about the processes and tools caseworkers need to provide coordinated services, such as:

  • The ability to collaborate with other providers and agencies across the human services spectrum
  • Access to social services-specific assessments and workflows
  • Cross-program sharing of best practices and benchmarks
3/14/18 1:00 PM (CST) Integrating Value-Based Care into County Health and Justice Systems

Value-based care continues to shape services for individuals with behavioral health and substance use treatment needs. While the need to receive whole-person care is no different for a justice-involved individual, the organizational needs for county health and justice systems vary greatly from that of human services organizations.

Join us for a webinar presented by this panel of experts:

  • Ed Eilert, Chairman, Johnson County Board of Commissioners
  • Tim DeWeese, Director, Johnson County Mental Health Center
  • Robert Sullivan, Director, Department of Corrections, Johnson County
  • Tom Stucke, Vice President and General Manager, Public Sector, Netsmart

This webinar will show you how your organization can develop a framework for integrating a value-based care model, ultimately reducing recidivism and improving individual outcomes. You’ll also hear about Johnson County’s experience in developing and implementing programs designed to effectively manage the behavioral health of their justice-involved population, as well as their experience participating in NACo’s Stepping Up initiative.

You’ll leave the webinar with steps you can take to implement value-based care and an understanding of how to:

  • Collect baseline data, track progress and analyze and report on individual outcomes
  • Support and manage transitions across jails, treatment and community programs
  • Stratify population views to assess recidivism risk and prioritize treatment needs
