Bundled payments, ACOs, health homes and risk-sharing mean value-based care is here to stay. To thrive in this new and ever-changing environment, home care providers will be required to assume more risk. Value-based care is designed to encourage doctors, hospitals and post-acute organizations to work together to improve outcomes and reduce re-admissions. The only way to achieve this goal is to improve collaboration and identify those at risk for de-compensation by reducing clinical inefficiency while providing care in the most cost-effective location.
Post-acute providers will be required to integrate with local health systems and other community-based providers. As part of these integrated care communities, all care providers will be asked to assume risk and be accountable for outcomes. The best partners will have the capability to return outcome data to their referral partners to maximize incentive payments.
