the very first call from a patient or referral source and
data collection is continued through discharge and
follow-up reporting.
Following national initiatives to reduce medication
errors, TIER was developed with a Computerized
Physician Order Entry (CPOE) process that feeds our
electronic Medication Administration Record (MAR).
These two tools have the ability to utilize bar codes
for both patients and medication blister packs. Our

reporting on missed medications, medication errors,
and adverse drug reactions.
