CASE MANAGEMENT
Insight Case Management is a comprehensive solution that enables case management staff to manage the application process, capture pre-admission diagnoses, define and document needs, manage correspondence, assign and manage caseloads, and track referrals. As a module integrated with the Insight EHR, the patient's Problem List is accessible within Case Management.
- Application – Caseworkers can document the application process, track certification dates, generate and track correspondence, and document the clinical and non-clinical issues and problem areas related to the case. Staff can use "needs templates" to define and assign specific case needs for follow-up. The case manager and supervisor are assigned during the
application process.
- Follow-up – As the case progresses, workers can create and record
follow-up activities and
case documentation.
- Referrals – Case managers can link patients to community resources. Community resource records can be searched by name, classification or keyword. The linked resource records include address information, a description of services offered, date restrictions (if any), contact and eligibility information, as well as keywords. Caseworkers can generate and track
referral correspondence.
Case Management provides dynamic views of caseload by caseworker, caseload by supervisor, cases by community resource, and open cases by case manager and/or community resource. All
system-generated correspondence may also be viewed within Case Management.
For referrals not involving case management, Referral Workflow provides a quick and easy method of documenting internal and external referrals. Integrated with the Insight EHR, the patient's Problem List is accessible within Referral Workflow. Referrals may be linked to clinical encounters; a single click will insert the encounter diagnoses into the referral record. Diagnoses, needs, services,
follow-up activities, internal and external referrals and case status may be documented and tracked in
Referral Workflow.
PROVIDER CHARTING
Insight Provider Charting incorporates the taxonomy and hierarchical structure of the Omaha System. The Omaha System documents client care from admission to discharge using a comprehensive, research-based, standardized practice and documentation methodology. Health departments that utilize the Omaha System for nursing care planning and documentation can incorporate their Omaha Care Pathways into the Insight Provider Charting module.
Provider Charting provides a hierarchical care planning and outcome framework. Clinical pathways and care plans are classified first by domain, then by problem within the domain. Problems are evidenced by signs and symptoms, and addressed by intervention schemes which are divided into the areas of:
- Teaching Guidance and Counseling
- Treatments & Procedures
- Case Management
- Surveillance
As the case progresses, activities are documented against the intervention scheme. Insight enables nurses to link activities to clinical encounters.
Provider Charting supports the Omaha System knowledge, behavior and status (KBS) ratings, which provide a powerful and standardized method of
outcome reporting.
A summary screen is provided to search and display Provider Charting records for the current patient or all patients, and for any combination of domain, problem, diagnosis and provider.
