Connected Care in Action
Below is an example of how several Netsmart software solutions facilitate connected care in a real-life scenario across a variety of care settings.
The Scenario
James Abbott is a 28 year old single college graduate with an eight-year history of Schizophrenia, paranoid type. When on medication, he functions well and is able to work and live independently. He does not have close friends, but he is active in a local chess club. James has a supportive family that lives nearby.
When he has permitted his family to be involved in his life, he has remained on his medication. He recently had a recurrence of symptoms, which are intrusive enough to require hospitalization. He had one previous episode of non-adherence and hospitalization at age 25. The County Behavioral Health Department has been treating James for the past eight years for Schizophrenia.
